Levels of Treatment Care

Nuwave Health Services conducts an initial assessment during the in-take process to determine the level of care needed for the
individual. Nuwave provides five different levels of treatment care. Then a trained counselor creates a treatment plan with the participant. The counselor meets with the participant weekly to update the goals and benchmarks on the plan. Nuwave also offers mental health and PRP services to its participants.

Level 1

Outpatient Program (OP)

Level 1 is used as a step-down from higher-intensity levels of care or for people who are stable, but are in need of monitoring. This level
involves less than 9 hours of services per week. People placed in Level 1 care should have supportive home environments, display stable physical and mental health and be free of significant withdrawal risks.

Level 2.1

Intensive Outpatient Program (IOP)

Level 2.1 offers addiction education and treatment components 9 or more hours per week. Participants learn coping skills to manage their emotional, mental and physical health and develop other necessary recovery skills.

Level 2.5

Partial Hospitalization Program (PHP)

Level 2.5 is also known as day treatment and involves 20-35 hours of clinical services per week. This intensive programming structure is appropriate for individuals with unstable medical and psychiatric conditions.

Level 3.1

Low-Intensity Residential Services

Level 3.1 is designed for people with minimal withdrawal risks and stable mental and physical health that is being appropriately managed, but who also require a structured, stable environment free of distractions. Participants learn how to deal with tasks of daily living, receive assistance with stabilizing substance abuse disorder symptoms and develop necessary recovery skills. This level provides at least 5 hours of clinical services per week.

Level 3.3

Medium-Intensity Residential Services

Level 3.3 takes place in specialty facilities and is intended for those with significant cognitive impairments related to their substance abuse disorder or co-occurring mental health disorder. Treatment may occur at a slower pace until the person’s cognitive abilities improve or until they
can progress to the next level of treatment.

Mental Health Services

Mental health includes our emotional, psychological and social well-being. At Nuwave Health Services we address the mental health of our participants with an integrated psychiatric rehabilitation treatment strategy. Our mental health team consists of psychiatric nurse practitioners, rehabilitation counselors, licensed mental health professional counselors, psychiatric rehabilitation specialists, social workers and occupational therapists.

Receiving a Diagnosis & Treatment Plan 

The first step in managing mental health is to receive an accurate diagnosis. Our psychiatric nurse practitioner will assess symptoms and make a diagnosis for each participant. After diagnosis, our mental health care team will help develop a treatment plan that may include medication, therapy or other lifestyle changes.

Medication Monitoring

Professionally overseeing the proper distribution of prescribed medication. Providing education to help individuals avoid the dangers of incorrect medication administration.

Independent Living Skills

Gaining knowledge and experience in key attributes of self-sufficient living that’s required to make it through everyday life. Training and development of critical abilities that promote healthy human interaction. Assistance with addressing entitlements.

Job Preparedness Training

Resume building, interviewing skills, as well as organized practice of societal and occupational skills and habits and assisting in obtaining employment.

Financial Literacy

Fundamental principles of managing money and rebuilding credit and receiving education that allows a individual to make informed and effective decisions with their financial resources.

Transportation

Nuwave Health Services will coordinate transportation services for qualifying participants and/or residents.

Psychiatric Rehabilitation Program (PRP)

Nuwave Health Services Psychiatric Rehabilitation Program (PRP) provides specialized services to treat individuals diagnosed with mental health disorders or individuals who have dual diagnoses in which chemical dependency co-occurs with mental health disorders. Group or individual counseling is provided in an on-site or off-site setting. Individuals are cared for by a professional team of staff members, which includes a Program Director, Licensed Rehabilitations Specialists and Rehabilitation Coordinators who are dedicated to providing the best care possible for all individuals in Nuwave Health Services.   

  

Our Psychiatric Rehabilitation Program (PRP) promotes recovery, full community integration and improved quality of life for persons who have been diagnosed with any mental health condition that seriously impairs their ability to lead meaningful lives. Psychiatric rehabilitation services are collaborative, person-directed and individualized. Our services focus on helping individuals develop skills and access resources needed to increase their capacity to be successful and satisfied in the living, working, learning and social environments of their choice during the recovery process.  
 

Psychiatric Rehabilitation Program (PRP) services provided include: 

Psychiatric (symptom management, relaxation, meditation, support groups and in-home assistance)

Health and Medical (maintaining consistency of care, primary care physician and mental health counseling) 

Housing (sober environments, supervised housing, group home living and apartment living)

Social (relationships, family, friends, housemates and boundaries, communications and community integration)

Basic Living Skills (personal hygiene, sharing meals and goal planning)
